This allows the hero to attack twice, are you allowed to split up the 2 attacks? Can you attack-move-attack again, or does it have to be 2 attacks on the same monster?

Since it doesn't specify we've always defaulted back to the general rule that all movement and all actions must be contiguous events unless an artifact or spell overrides it. This same interpretation of not dividing up moves/actions would apply to the Orc's Bane and Elven Boots too.

As a curious aside, the Barbarian Quest pack outlines multiple attack rules for Monsters (where Heroes only get to defend once) but there's no such rule for Heroes that have multiple attacks ;)

I reworded my heroic brew so that it allows an additional attack action.

It's clearer than to say "attack twice", exactly because of that question of you.

It also means you can drink ten heroic brews and attack ten times, but I accept that because there are not so many heroic brews in my deck.

"Heroic Brew Addiction: You've become addicted to Heroic Brew! Without it, you are worthless. If not using Heroic Brew, you cannot roll for any attack. Anytime you have the opportunity to do so, you must Search for Treasure, in hopes of finding yet another Heroic Brew potion."

And after 20 Heroic Brews your liver fails it you overdose. (You must finish the quest before succumbing to it's afflictions)

After succumbing to its effects, your stats revert to those of a Zombie. Only an Exorcist can save you now...

Attack is DURING the action, so for me "attack twice" means that you can perform 2 attacks during the SAME action.
You don't earn an extra action, you earn the extra attack and it must be performed during your only action. So you can not attack-move-attack, it would be like action-move-action.

That's a nice interpretation!

It would also scream for a rewording of the card. "In your next action, you may attack twice".

Yes, there are additional implications for both formulations. The original rules ship around the problems by staying ambiguous.

* My additional attack action is clearly better for the hero in most cases, as it allows attack move attack for example. On the other hand, effects which last for one action are worn off in the second action.
* Lesto's two attacks in one action allow no move in between, however if an effect lasts for the next action, it can be used for both attacks.

On the original card and game system, you could interpret both. The original rules have not been clear about that at all; especially the "Action" is mentioned only in the rule book, not on the cards.
For example the Potion of Strength tells you to roll 2 extra dice "next time you attack", omitting the "action" word completely, resolving both Lesto's and my variant so that it works only once. Same for some other effects.
